THUNK.
Huge Army, better stats than you'll ever have, dimension door, serious attitude, sphere of negation preventing spells in combat.
And all you get to fight him with is one hero, though strong, always weaker than THUNK, with human troops, and if that hero dies, you lose the game.
As for grunts, I'll add another vote to those intelligently acting poison spitters from Dialbo 1.
I beat the entire game without using healing poitions, just scrolls of town portal, and they were by far the most difficult section.

The Cyberdisks in X-COM were just nasty. When you saw them, you knew that someone was going to die. Their plasma attack burned right through even the best armor, and they self-destructed with a massive explosion when they died. Killing them gave a sense of accomplishment.
As for the BBEG (Big Bad Evil Guy/Gal), I'd vote for Chrono Trigger's Lavos. All console RPG villans want to destroy the planet, but they usually have some stupid metaphysical reason for doing so. Lavos was just a big hungry monster, and that was cool. I liked the way they set up that enemy.

Big boss: Baphomet/John Romero's head on a stick, Doom II. (The big skull is never given an actual name in Doom II, but Final Doom calls it "Baphomet," so I just go by that.) Honorable mention to Magus, Chrono Trigger. Practically everything in the game up to that point implies Magus, not Lavos, is the last boss.
Normal boss: Duriel, Diablo II.
Grunt: Warmech, from Final Fantasy I. Honorable mention to the minions of any CEB archer, Diablo II, v1.10. (The CEB archer itself I don't have much trouble with. It's the fact that the attacks of the minions are cold-enchanted, as well...)

Regular Shmoes:
Earthbound:
Those trees in the early part of the game that always explode and kill somebody. Any battle with those Oak Trees meant serious PP usage for healing.
Boss:
Final Fantasy 1
Warmech
On the walkway to Tiamat, the boss of the tower section, you had a 1 in 64 chance of encountering this monster. Unfortunatly, my first time happened to be it. Massive number of HP (rivaling the final boss of the game), massive number of spells (all level 9: Nuke, XXXX!, etc), and even worse, always went first. If you beat him (it?) handily, you could easily beat the rest of the game.
Edit: Did not see the above post. Wow, somebody else encountered the 'Mech!

Holy cow, how could I have forgotten about him! And that was my favorite game at the time.
Best (worst?) of all, he was indestructible! You either had to go on to the next screen or perish.
