Regions are a great addition, IMHO. Here's how they work:
You start playing in one city. As is normal in Sim City games, you create road/rail/highway etc. connections to your neighbord. In SC4, they mean something. Traffic and jobs will pass from one city to another.
When you create a second city, the connections made to it from the other city will appear on the borders. Time will not have changed- if you played 10 years in one city the others will still be at 0 years. What changes is, well, just about everything else. The most obvious factors are RCI demand and cash. RCI is affected by neighboring city population. A high residental area will raise demand for industry and commercial in adjoining cities, same for commercial etc. Neighbor deals mean something too- if you are buying power from one city, it must have connected power lines and enough plants to support the demand. Your income (or cost) will change as the base price changes- ex. higher $/kwh plants means you will be paying more (or getting more income).
Traffic is also a calculated factor now. Hitting alt-? and clicking on nearly any structure will show you how people travel. Alt-clicking on roads will show where the traffic is going to and from. You will also see the types (car, bus etc.).

The growth continues to boom. Farms have been destroyed to make way for urban growth, and Jonnyopolis is now close to reaching maximum capacity. Traffic is becoming ever more of a problem; roads have been turned to avenues, the rail system has been expanded, a bus line has been implemented, but the problems remain. But it must be a good city nontheless, otherwise there wouldn't be 102,000 people living there. In the future, we are planning to expand the bus system citywide, build an airport in the cleared area on the east side, expand the education system to more than just elementary schools, build a highway, and possibly later on build a subway system.
