I had taken 1 year at University, with nothing before that. I'm the same, My vocabulary stunk but I was good in the other areas.
When I was taking it, My vocabulary was sooooo bad (s*cks actually), that I was about to fail my first semester. Towards the end, I started making up German Flashcards, with the German words on one side (and their different conjigations/pluralisations/etc) and the english word/translation on the other side.
I didn;'t start my flashcard projuct until about 1/2 through the semester and they helped me bring a badly failing grade up to a high D. The next semester, which was tougher, I got a B.
It Took me forever and a LOT of 3x5 cards, but once I had them done, I could just suffle and work through them, when I had the freetime. And I could practice both German to English and English to German. I would work at going through the whole deck at least once per day.

The other option is, as I said above in German, to talk German or write with Germans. That is how I learned English. My notes for English were a mean of a 2, that's an F for Americans. So I dropped the subject at haighschool (under the former law that was permitted) and took German instead and did for the language subjects/courses only exams in German and of course Dutch.
When I became member of Apolyton, I knew of the basic English grammar, but very few words, so I had to look up almost every word in my dictionary for it's meaning. Often I had to look up a word more than once, but I remembered it in the end. Slowly but steadily I learned English.
Right now I only have to look up very special words. My English is even so developped that I can make poetry in English and I made and attempt to write some prose.
