I just got a chance to listen to Xenosaga Ep. II's music recently, and I was a bit underwhelmed. Kaijura did WONDERFUL work on Noir & .hack; XS II was merely good, and I had gotten used to Mitsuda.
But yeah. If we're sticking to PC games, I'll say Heroes of Might & Magic has always had pretty good music. On consoles, we have Uematsu (already mentioned for Final Fantasy series) & Mitsuda (Xenogears, Chrono games, Xenosaga, a bit of Shadow Hearts) as two obvious notables... also very good composers would include Motoi Sakuraba (Star Ocean, Valkyrie Profile, Golden Sun, Baten Katios... closest thing to a progressive rock video game composer around) Michiko Naruke (Wild ARMs series), Yoshitaka Hirota (Shadow Hearts & SH2- a video game counterpart for Taku Iwasaki, who did fine anime music in a similar style), Yoko Shimamura (Parasite Eve, Legend of Mana), Michiru Yamane (Castlevania series, other stuff), Noriyuki Idaware (Lunar series, Grandia series) & Jeremy Soule (KOTOR 1 & 2, Total Annihilation, Secret of Evermore). Hmm, that was longer than I intended... I suppose I give away that I listen to entirely too much video game music in my spare time.

Hitoshi Sakimoto (YmoH.S for really old geezers), done things like Ogre Battle and Vagrant Story, will do FFXII, Masaharu Iwata (REZON), mostly same games as Sakimoto, Junya Nakano, much of FFX...
Yoko Shimomura, (Parasite Eve, Kingdom Hearts, Front Mission), Bunbun (Megaman III), Koji Kondo (Super Mario games), whoever did the musics for Nintendo World Cup on NES... 
Jun Ishikawa (Kirby3), Michael Giacchino (Medal of Honors), Miyoko Kobayashi, Masanori Hikichi (Terranigma), Yasunori Mitsuda (Radical Dreamers), Follin brothers (Tom&Jerry on NES, Bionic Commando on C64)
Allied General on the PSX had good music. As does Panzer General 2 on PC. And the Syndicate theme rocks. And Command&Conquer had good music.
And Jeremy Soule for Secret of Evermore.
