Overall I liked the episode. I thought it hit a good balance of revealing things and still making me say WTF???

I agree. I can't see them keeping the mystery and eerie feeling for 5-6 seasons. People would likely get peeved and stop watching.

I do. To me it shows that they do get these types of things rights from time to time but I still shudder at the idea of them having to come up with whole new lineups every year

Hopefully shows like "LOST" learn from that, but economics usually dictate that the show keeps going for as long as it is making good money, which is usually for about one season after the quality declines

Okay, here's some speculation--
spoiler(highlight to read): Desmond isn't the only person who is living, or has lived, in the bunker. There are bunk beds, a ping pong table. Perhaps Ethan was an occupant? Moreover, the person in the beginning when we hear the hatch blow is showered and clear; Desmond looks rather ragged at the end. Some time did pass in between, so it could still be him, or it could be that the beginning was a flashback-sorta thing of Desmond's happier days in the bunker. It did start with a close-up of his eye, after all.
Also, the bunker was sealed from the inside with the word "quarantine" because the island is dangerous, not the bunker. The meds he was giving himself ward off the infection that creates The Others.
Did he just end up on the island during this "race around the world" he was training for? Did he just stumble upon the bunker? How did he get in? He apparently arrived on the island sometime after Rousseau and her crew (not to mention the Black Rock), so what does he know of her and vice versa?
