The happiness factor is the biggest difference with Deity, IMHO, Raz. You have to monitor your cities closely, anticipating when unhappiness will arise and acting accordingly. Build another military unit or bring an Elvis into play. You can build well beyond the limit (of cities before revolt) in Despotism and Monarchy providing you monitor them closely and anticipate accordingly. And, of course, each city is contributing at least one trade arrow to the pile.
If the AI civs are finding you that early on a large map, you're a bit stiff. Start again. I've often played for many hundreds of years - sometimes thousands - before I've met another civ on a large map.
What's your order of research? If you haven't explored much before starting your first city, go HBR first off. Handy for defence purposes - particularly if you start with only warriors - and even more useful for exploring.
