Normally the odds of planting a spy are 50%, according to the Strategy Guide. Repeated attempts on the same turn are less likely to suceed, but I don't know by what percentages. The failure is noticed by the AI being targeted, too. Perhaps a few gifts and a short wait are the best idea, before trying again to insert a spy. Once the spy is planted, make sure you have a big pile of gold to take along to finance the spy's missions. They are all quite pricey.

I don't think power and size matter, it's just a 50/50 chance like the strategy guide says. In one game I failed planting a spy with a small civ, but later on the same turn, suceeded in planting one with the most dominant civ in the game.
