I've tried making repeated demands. The AI refuses, but gets real annoyed.

2 options:
1. Diplomatic: keep making insane demands (money, tech, cities), then say "take this deal or suffer!", though this only worked for me once. Another way is to refuse any demands from the AI, from "fair" (to them) trades, to threats. That will get you a war faster.
2. Pretend to be weak: leave your borders undefended. This works if you also leave a patch of unsettled land on the opposite side of your empire. Sooner or later, AI settlers will start pouring over your border. To my knowledge, the AI always escorts settlers with a defensive unit. If you keep nagging the AI to remove its troops, that defensive unit will eventually give you the option to say "remove your troops or declare war!". Even with this method, you will have to wait a few turns, but it's the fastest way you can do it.
