The lighting doesn't look quite right. You can see the shadow his head leaves but the rest of him doesn't seem to cast anything.
The head looks fractionally oversized too but I may be imagining it...

I'd say the pic is real, it's just the man on it is someone that looks like Saddam, not necessarily the man himself.

The shadow is fine - it's just a result of poor use of flash, if you ask me - I got a ton of pics like that when I didn't know how to use the camera properly.

EDIT: On the second look, it does seem that the head and the neck don't match very well (like Calc says) in that the head seems a little twisted to the side, but not the neck...

The man's head-neck is p shaped for crying out loud. Obviously the person did a spectacular job lingin up the neck with back of the head, but it makes saddam's head look like a bean sprout as a result..

I would have resized his head, but I have a feeling that the shadow was from a real person's head, which is why he went for saddam's head the way it is.

Yeah, but you have to agree, that the pale face and the red-eye effect go very well with the shadow created by the flash being too bright! (Unless, that is, you assume Saddam is a vampire )

1. His head is large.
2. The lighting angle and the angle of his shoulders prevents most of the shadows from showing as much as the shadow of his head.
3. Could still be a double, either an official Iraqi double or someone in the West who happens to look like Saddam.
