Have you read the readme file I included with the .zip?
If you have vanilla civ: unzip this into your Civ3\Art\Smallheads\ folder and agree to overwrite the file there (before you do this, you should back it up by copying it elsewhere).
If you have PTW, either do the above, or create a new folder in Civ3\Civ3PTW\Scenarios\ and call it anything you like ("X" for example). Now create a subfolder X\Art\Smallheads\ and copy the popheads.pcx included in the zip to that subfolder. Then create a new scenario X.bix, and that should be more or less it. If it still doesn't work, ask here and I will try to help you (provide detailed descriptions of what you have done).
