1) A civ can split in to two when (a) it is the most powerful civ (as measured by the power graph) and (b) its capital is sacked by another civ. This doesn't always happen; there's a randomizing factor involved. But that's the only condition under which it can happen. There may be a way to make it happen all the time by fiddling with rules.txt, but that's beyoind me; I'm sure someone else knows, however.
2) It seems as if civs will restart only up until a certain date, but I've never seen a definitive answer as to which date (1000 AD feels right to me, but that's a wild guess). Also, civs will only restart if there's a decent place to do so -- inhabitable land at least some disatnce from the other civs. So, in a game with a lot of early expansion on a small or medium map, it's possible that even before 1000 AD (or whatever the date cutoff is), there'd be no place to restart. Of course, you can always keep civs from restarting altogether by selecting "no restart" on the custom rules menu at the start of a game.

I have seen in a test (autoplay-no human)that the weakest civ (3 cities) of 3 civs also splits by losing the capital to the strongest AI-civ.

I have restored an old savefile.
Open, center map on Tenochtitlan. Aztek civ will split in a few turns. Germans or americans will conquer the city. germans has better chances of doing this.
It was a testgame, so cheating is enabled.
I tried to provoke an worldwide nuklear war.
